The Vision Support Template is a handy tool that can act as a roadmap, guiding teachers in understanding the specific needs of each student and implementing appropriate accommodations and modifications.

While updating my forms, files, and worksheets for the school year, I wanted to share with you the vision support template that I use to give to my classroom teachers, support staff, and therapists working with my students who have visual impairments. It is similar to this one posted on the Perkins website, Vision at a Glance Template. 

Understanding the template’s purpose

The Vision Support Template is designed to be a comprehensive and easily accessible document, providing essential information about a student’s visual impairment and the strategies needed for their success in the classroom. This template, often prepared by a Teacher of Visually Impaired (TVI), outlines crucial details that empower general education teachers and support staff to effectively support their visually impaired students.

Utilizing the template effectively

The Vision Support Template is not a static document but rather a dynamic tool that should be utilized and updated regularly. Teachers should:

  • Review it frequently: Especially at the beginning of each school year, to ensure familiarization with the student’s needs and the outlined supports.
  • Share with relevant staff: Share the template with all teachers, substitute teachers, and support staff who work with the student to ensure consistent implementation of accommodations and modifications.
  • Collaborate with the TVI: Maintain open communication with the TVI and O&M instructor, who can provide further guidance and support in adapting strategies as needed.
  • Involve the student: As appropriate, involve the student in the process of reviewing their Vision Support Template and empowering them to advocate for their needs.
  • Adapt based on progress: The effectiveness of accommodations and modifications should be monitored and adjusted as needed, based on student progress and evolving needs, keeping it a live document.
